Окно автоматически не закрывается, из кода ясно, что должен быть редирект, но его нет. Целевая страница не обновляется, пользователь не понимает, что уже авторизировался.

При повторной авторизации всё норм.

Судя по скриншоту у вас не корректно настроен веб сервер.
По всей видимости сервер отдается страницу как текст.

При повторной авторизации идут несколько другие механизмы, по этому проблем не возникает.

Проблему решить не удалось. Подскажите куда копать? Возможно какая то надстройка php?
